CBT for Obsessive–Compulsive Disorder (OCD)

Educational content only. ERP is most effective with trained guidance. See our Medical Disclaimer.

Overview

OCD features obsessions (intrusive thoughts/images/urges) and compulsions (repetitive behaviors/mental acts) aimed at reducing distress. Compulsions provide short‑term relief but reinforce the cycle.

Why CBT helps

  • Exposure and Response Prevention (ERP): planned exposure to triggers while resisting compulsions to relearn safety.
  • Cognitive work: normalizing intrusive thoughts and revising inflated responsibility/over‑importance of thoughts.
  • Values focus: acting toward meaningful goals instead of compulsive rules.

Journaling prompts

  1. Trigger → urge → chosen response (allow urge, delay ritual). Peak anxiety __/100, duration __ minutes.
  2. What did I learn about my capacity to tolerate uncertainty without ritualizing?
  3. One values‑aligned action I took instead of a compulsion was…

Use brief logs in How to Journal.

When to seek care

ERP is highly specialized; a clinician can tailor hierarchies and ensure safety.
