CBT for OCD — Harm (ERP)

Overview

Harm OCD features intrusive images/urges about harming oneself or others and rituals like avoidance, mental neutralizing, or seeking reassurance. The goal is to learn that thoughts are not actions.

Why ERP helps

  • Approach feared cues: handle safe objects (e.g., kitchen knives) during normal tasks.
  • Script exposures: write/read feared thoughts without neutralizing.
  • Response prevention: reduce checking and reassurance.

Example exposures

  • Cook with a loved one present while holding a knife; label thoughts as “just thoughts”.
  • Write a brief script of the feared scenario; read it aloud daily without neutralizing.
  • Stand near an open window and notice thoughts pass without safety rituals.

Journaling prompts

  1. Trigger → obsession → ritual urge → what I did instead → learning.
  2. Peak anxiety __/100 and time to decline by half __ minutes.
  3. Evidence that thoughts are mental events, not intentions.
