CBT for OCD — Symmetry/Ordering (ERP)

Educational content only. ERP is best delivered with trained guidance. See our Medical Disclaimer.

Overview

Symmetry/ordering OCD involves intense discomfort when items feel “not just right” leading to arranging, counting, or repeating until a sense of completeness appears.

Why ERP helps

  • Misalignment exposure: intentionally misplace or leave items uneven.
  • Response prevention: ride out discomfort without fixing or counting.
  • Flexibility training: practice “good enough” responses.

Example exposures

  • Leave books slightly uneven on a shelf; do not correct for the day.
  • Wear socks that are not perfectly aligned; continue routine.
  • Place a picture frame a few degrees off level; resist fixing.

Journaling prompts

  1. Misalignment I created → discomfort peak and decline → what I learned.
  2. Which safety behaviors did I skip (counting, straightening, repeating)?
  3. One flexible thought I practiced (e.g., “good enough is enough”).

See How to Journal for structure.

When to seek care

Complex rituals or high impairment benefit from clinician‑guided ERP.
